Physical rehabilitation of patients after heart transplantation

In recent years, the number of heart transplantations (HT) has continuously been increasing in Russia. In 2015, 179 heart transplantations (1.2 per 1 million population) were performed in Russia in total. Consistently, the number of patients managed by cardiologists after successful HT is increasing, and the cardiologists face issues of administering adequate physical rehabilitation and work capability medical expert assessment to such patients. This review presents mechanisms of impaired exercise tolerance after HT and effects of physical rehabilitation on functional condition, quality of life, and prognosis of these patients. Different types of exercise training and their effects on patient rehabilitation to their original physical activity are discussed in detail. The review also focuses on programs of physical rehabilitation for patients included into a HT waiting list, and specifically patients with implantable assisted circulation devices. Information about psychosocial issues and work capability medical expert assessment in patients after HT is also presented.
