Peculiarities of left ventricular myocardial strain in patients with ST segment elevation myocardial infarction after revascularization

Background. One of methods for evaluating the LV inotropic function after revascularization is two-dimensional strain measurement or two-dimensional speckle tracking (2DST). Aim. Dynamic evaluation of myocardial strain characteristics in patients with ST segment elevation MI (STEMI) with different outcomes of revascularization using 2DST. Materials and methods. The study included 49 patients with STEMI and a control group consisting of 21 healthy volunteers. Based on the TIMI scale evaluation of revascularization effectiveness patients with STEMI were divided into two groups; group 1 included patients with angiographically good results (TIMI 2-3) and group 2 – with unsatisfactory results (TIMI 0-1). EchoCG was performed for all study participants. Results. According to data of 2DST at 3 months of follow-up, groups 1 and 2 showed a tendency to improved LV global longitudinal strain. Furthermore, values of circular strain increased with time in patients with good and satisfactory revascularization whereas in group 2, this value did not significantly change and rather values of radial strain increased. Conclusion. LV myocardial strain characteristics to a high degree depended on the revascularization outcome in patients with STEMI. Analysis of all types of global deformation in quantitative terms based on results of the 3-month follow-up provides insight into both the nature of myocardial injury and myocardial compensatory mechanisms among which global radial strain is perhaps the major one.
