Effect of immune inflammation on arterial stiffness and central pressure in women with osteoporosis in combination with IHD

Background. Osteoporosis is associated with a high risk of CVD. The community of osteoporosis and atherosclerosis is under discussion. Systemic inflammation underlies progression of atherosclerosis. Pulse wave velocity (PWV) has been demonstrated to be an independent CVD RF, and the pathogenesis of arterial stiffness is extensively discussed. This determines the interest to studying arterial stiffness parameters and their relationship with immunological markers in patients with cardiovascular pathologies and comorbidities. Aim. To establish the interrelation between parameters of immune inflammation and arterial stiffness in patients with osteoporosis comorbid with IHD. Materials and methods. 98 women with IHD (age, 71.2±8.6) were evaluated. Group 1 (50 patients) consisted of women with isolated IHD (age, 71.4±8.4); group 2 (48 patients) consisted of patients with a combination of IHD and severe osteoporosis (70.9±9.1 years). Levels of osteoprotegerin, transforming growth factor β (TGF-β), interleukin (IL) – 1β, IL-4, IL-6, IL-8, IL-10, TNF-α, soluble type II IL-1 (ILsR II) and TNF (sTNFr) receptors were measured in serum using the immunoenzyme assay. PWV and central pressure were measured using applanation tonometry on the apparatus SphygmoCor AtCor Medical, Australia. Results. In women with the comorbidity, levels of IL-6, – 8, – 10, TNF-α, and osteoprotegerin were increased, and levels of ILsR II and sTNFr were decreased. Increased PWV, aortic pulse and diastolic pressure, and the augmentation index were observed in patients with osteoporosis comorbid with IHD. In women with osteoporosis comorbid with IHD, independent determinants of increased arterial stiffness included age, IL-6 level, absolute risk hip fractures (HF), aortic pulse pressure, and augmentation pressure. Conclusion. in women with osteoporosis comorbid with IHD, levels of some cytokines, parameters of arterial stiffness and aortic pressure were increased. Independent factors of increased PWV included age, HF, aortic pulse pressure, and augmentation pressure.
