Diurnal changes of the thyroid function in patients with stable ischemic heart disease and persistent atrial fibrillation

Background. Close interrelation of the thyroid state and cardiovascular diseases (CVDs) is well known. Presently, overt thyroid abnormalities are rather well studied whereas most of unsolved issues are related with latent thyroid dysfunction. Obviously, even a slight change in diurnal thyroid activity can considerably influence development of various diseases, including CVDs. Therefore, the interrelation between CVDs and thyroid dysfunction, specifically disorders of thyroid rhythmic activity require special investigation. Aim. To study interrelation between 24‑h secretion patterns of thyrotropin and free triiodothyronine (fT3) and clinical features of CVD. Materials and methods. Levels of thyrotropic hormone (TSH), fT3, and plasma cortisol were measured twice daily (at 8.00 and 18.00) in 65 patients with IHD associated with arterial hypertension (AH), persistent atrial fibrillation (AF), and without heart rhythm disorders. Considered parameters also included duration of IHD and AH; presence or absence of AF; incidence of arrhythmia; time since the last arrhythmia episode; and EchoCG data. Results. Diurnal changes in TSH level were rhythmic; TSH reached high values in the morning and low values in the evening but did not exceed the normal range. The fT3 level was characterized by inversion of fluctuations during a day. The pattern of described changes depended on the presence of AF whereas the range of diurnal changes in TSH and fT3 was related with the time since the last episode of arrhythmia and duration of the history of hypertension.
