Diabetes mellitus and arterial hypertension: rational antihypertensive therapy

Relevance. Selection antihypertensive agents in patients with DM type 2 is very significant as this disease imposes a variety of restrictions to administration of various medicines. Objective. Evaluation of Prestance effect (combination perindopril and amlodipine) on 24‑hour BP profile, carbohydrate and lipid exchange, progression of diabetic nephropathy in patients with DM type 2 and AH. Materials and methods. 57 patients with DM type 2 and AH were under dynamic supervision. After wash-out period (patients did not took antihypertensive therapy for 5–7 days), patients took Prestance (5 / 5, 5 / 10, 10 / 5, 10 / 10 mg) for 6 months. BP, carbohydrate and lipid exchange, renal function were supervised. Results. Due to administration of the drug, target BP level was achieved in 98 % patients with AH and DM type 2, stable 24‑hour control of BP level. Cholesterol, LDL levels significantly decreased and HDL level raised, microalbuminuria level significantly decreased ( – 35.5; р<0,005). Conclusion. High antihypertensive treatment efficiency of Prestance is combined with beneficial effect on renal hemodynamics, carbohydrate and lipid exchange.
