Time-related changes in left ventricular diastolic function in the most acute period of myocardial infarction

Background. LV diastolic dysfunction (DD) is one of the earliest predictors for HF particularly in patients with acute MI. Time-related DD changes in the first hours of the disease are understudied. Aim.  To study time-related DD changes during the first day of acute MI. Materials and methods. 61 patients with Q-positive MI were evaluated after thrombolytic therapy (TLT). The diastolic function was assessed using cardiac ultrasound in the first hours of IM prior to and at 6 and 12 hours after thrombolysis. Impaired diastolic function
was observed in the majority of patients (94.7 %); restrictive DD was found in one-third of patients (33.9 %) at the first examination. Following TLT, the proportion of patients with restrictive DD decreased to 5.3 %; furthermore, more than a half of the patients (51.8 %) had grade 1 DD. Conclusion. Thrombolytic therapy considerably reduces the DD severity also in patients with the most unfavorable, restrictive DD.
