Daily changes in parameters of thyroid status in patients with ischemic heart disease, atrial fibrillation and chronic heart failure

Background. Functions of the cardiovascular and the thyroid gland (TG) systems are known to be interrelated. Of special interest is the effect of occult TG abnormality on development and course of CVDs. Aim. To evaluate the interrelationship between diurnal secretion patterns of thyrotropin (TT) and free triiodothyronine (T3f) and clinical courses of CHF and atrial fibrillation (AF) in patients with IHD. Materials and methods. Plasma levels of TT, T3f , and cortisol were measured twice daily (at 8.00 a.m. and 6.00 p.m.) in 75 patients with IHD and AH complicated by CHF in combination with persistent AF or without heart rhythm disorders. The following parameters were recorded: presence or absence of AF; AF rate and time since the last episode of rhythm disorder; CHF severity; and EchoCG data. Results. In patients with IHD complicated by CHF, changes in plasma TT were consistent with changes in cortisol level and reached high values in the morning and low values in the evening without exceeding the normal range. The level of T3f was characterized by inverted fluctuations during a day. Diurnal changes in the thyroid status of IHD patients depended on definite clinical characteristics. Significantly more narrow ranges of TT and T3f diurnal changes were observed in patients with AF. Changes in TT and T3f also depended on CHF severity.
