Possibilities of using the ultrasound method for evaluation of myocardial deformation in heart failure with preserved left ventricular ejection fraction

Traditionally CHF is divided into systolic and diastolic forms. The systolic CHF form is associated with reduced LV contractility and LV EF below 40 %. The diastolic CHF form is characterized by impaired relaxation and filling, loss of elasticity, and increased stiffness with preserved LV EF (>50 %). Such cases can be classified as HF with preserved LV EF (HF-PLVEF). Prevalence of such HF-PLVEF patients depends on CHF severity in the studied population and criteria for evaluation of LV myocardial contractility. Since modern digital technologies were implemented in clinical practice, a new method became available for quantification of global and regional LV contractility. The method is based on measuring the velocity of myocardial deformation by tracking speckles (natural acoustic markers) on a standard 2D EchoCG image of the myocardium on grey scale B-mode images during the entire heart cycle. The method was named speckle tracking echocardiography (STE) or an ultrasound method for evaluation of myocardial deformation.
