Significance of troponin tests for differential diagnosis and prediction in heart failure

Heart failure (HF) is one of diseases associated with increased blood concentrations of cardiac troponins. The review focuses on current reports of diagnostic and prognostic significance of troponin tests in acute HF and CHF. The issue relevance is supported by the fact that HF is the major cause for over-diagnosis of type-1 MI according to the analysis of fatality performed by the authors in patients with vital serial measurements of troponin at two independent Moscow hospitals. The review provides aspects of differential diagnosis for increased cardiac troponins in type-1 MI and acute HF; assessment of the predictive value of troponin tests for short- and long-term prognosis in patients with acute HF and CHF based on the type (T or I) and sensitivity of troponin tests, timing of measurement, and the used threshold value; and analysis of European and American guidelines for mentioning the use of troponin tests in HF.
