Morpho-functional parameters of the heart and features of immunity shifts in patients with chronic heart failure induced by dilated cardiomyopathy

Background. Despite versatile studying the issues of detection and features of immunity disorders in myocardites and dilated cardiomyopathy (DCMP), a balanced opinion on criteria and diagnostic methods for these conditions has not been developed yet. Issues of interclinical relationships; inflammatory activation and severity of non-ischemic CHF; and increased production of proinflammatory cytokines and antibodies to myocardial tissue in DCMP as interrelated with changes in cardiovascular clinical and morpho-functional parameters have remained understudied or even open. Aim. To study the relationship of inflammatory markers (CRP), proinflammatory cytokines (TNF-α, IL-6) and antibodies to myocardial tissue with clinico-functional parameters of CHF severity in patients with DCMP. Materials and methods. 80 patients with DCMP with clinical manifestations of NYHA FC I–IV HF and 29 healthy volunteers (control group) were evaluated. In addition to evaluation of medical history and the clinical and immune status, EchoCG and X-ray cardiometry were performed. Data were analyzed with relation to severity of CHF course and DCMP variant. Results. Significant increases in blood levels of proinflammatory cytokines and inflammatory markers and high peripheral blood levels of antibodies to myocardial tissue were observed in DCMP patients compared to the control group. Concentrations of TNF-α, IL-6 and CRP were the highest in patients with FC IV CHF and signs of cardiac cachexy. The increased titers of antibodies to myocardial tissues were an early diagnostic marker for DCMP. Conclusion. The study identified immune predictors of early DCMP and severity of its clinical course.
