Diagnosing chronic heart failure in climacteric women complaining of shortness of breath

Background. A considerable part of female patients complain of shortness of breath. The most common causes include obesity or overweight in combination with low physical activity and detraining. However cardiovascular disorders may promote CHF. Aim. Analyzing the incidence of CHF development in postmenopausal women visiting a doctor for shortness of breath. Materials and methods. The cross-sectional study included 151 postmenopausal women of those who visited a doctor for menopausal symptoms and complained of shortness of breath. For all women, antropometric measurements were performed, the modified menopausal index (MMI) was evaluated, brain natriuretic peptide (BNP) level and parameters of lipid and carbohydrate metabolism were measured, and echoCG and 6min walk test were performed. Results. 120 women (group 1) had normal BNP levels. Group 2 (with high BNP) included 31 women. All female patients had obesity or were overweight. In both groups, disorders of lipid and carbohydrate metabolism were observed, which were more pronounced in women without CHF. Hyperinsulinemia was found in the group with high BNP. Patients of this group also had signs of insulin resistance (IR). EchoCG showed that LV EF was within the normal range in all patients. Conclusion. CHF was found in each fifth postmenopausal woman complaining of shortness of breath. Measurement of BNP helps differentiating between the causes of shortness of breath in women with abdominal obesity. The development of CHF is associated with pronounced IR.
