Interrelation of endotoxemia, factors of the systemic inflammation and components of the matrix metalloproteinase system – tissue inhibitors of metalloproteinase at chronic heart failure

Relevance. The fact of immune system activation in CHF patients can now be taken for granted, however, its causes and consequences at this pathology demand further studying. Objective. Study interrelation endotoxinemia, systemic inflammation action and condition of components of the system of matrix metalloproteinases (MMР-9) and tissue inhibitors of metalloproteinases (TIMP-4) at CHF in patients with postinfarction cardiosclerosis (PICS). Materials and methods. 230 patients with PICS underwent EchoCG, blood plasma levels of N-terminal pro b-type natriuretic peptide (NT-proBNP), endotoxin, tumour necrosis factor – α (TNF-α), C-reactive protein (CRP), MMР-9 and ТIМР-4. Results. Strong correlation between endotoxin levels and inflammation factor concentrations was revealed: for TNF-α correlation factor is 0.71, CRP – 0.82 (both р<0.01). Concentration of MMР-9 is positively correlated with TNF-α (rs = 0.48) and CRP (rs = 0.52; both р<0.01), concentration of ТIMP-4 is negatively correlated with TNF-α level (rs = –0.47) and CRP (rs = –0.53; both р<0.05). Conclusion. The results of this study allow considering that HF conditioned stagnation of the systemic circulation creates conditions for the penetration of bacterial endotoxin from intestines to the blood, which stimulates production of the systemic inflammation factors that in turn cause MMP-TIMP disbalance, which promotes pathological remodeling of the heart and CHF progression.
