Survival of patients with chronic heart failure according to the cohort study of a representative sample in the Ryazan region

Background. Due to increasing of CHF prevalence it is relevant to evaluate survival in this population and assess factors of adverse outcome. Objective. To study the survival and evaluate factors of unfavorable prognosis in patients with CHF FC I–IV, identified by screening a representative sample of the Ryazan region. Materials and methods. The trial was conducted in the framework of National epidemiological trial «EPOCHA». 167 patients with CHF FC I–IV had verified diagnosis on the basis of clinical assessment and echocardiography. Monitoring of vital status was carried out prospectively over four years. Results. During the period from 2002 to 2007 five-year all-cause mortality among patients with signs of CHF I–IV FC (22.7 %) was higher compared to examined population (8.9 %) and patients with AH (16.0 %). Four-year survival of 167 patients with verified CHF I–IV FC was 76.6 %. The main factors of unfavorable prognosis were age and CHF FC. Left ventricular ejection fraction as an independent clinical and instrumental predictor of survival lost its prognostic significance after multivariate analysis of clinical and instrumental parameters. Thus, four-year survival of patients with CHF I–IV FC was 76.6 %. Main factors of unfavorable prognosis in patients with CHF are age and CHF FC.
