Clinical and demographic characteristics of patients with chronic heart failure and implaned cardiac resynchronization therapy devices: data from the register PANORAMA in Russia

Background. Adequacy of principles of evidence-based medicine requires assessment of comparability of data obtained in multicenter randomized controlled trials in real clinical practice. Objective. To evaluate clinical and demographic data of patients with implaned cardiac resynchronization therapy (CRT) devices in real Russian medical practice according to the register PANORAMA–RUSSIA compared with similar data of large multicenter studies and international registers. Materials and methods. The study included 358 patients with implanted CRT devices, including clean-CRT (211 patients) and CRT in combination with defibrillation (CRT-D) – 147 patients. Results. In comparison with foreign studies Russian cohort is significantly younger (mean age 53±13.5 years), with higher prevalence of men (83.5 %) and non-ischemic myocardial damage (61.1 %). Less patients in Russia had a history of diabetes (11.4 %), hypertension (43.7 %), coronary arteries interventions (frequency of aortocoronary bypass in anamnesis was 15.4 %, transluminal balloon angioplasty – 8.7 %), valve surgery (5.1 %) and lower frequency of anamnestic MI (39%). Atrial fibrillation was revealed more often in Russian patients (39.9 %). Clinical severity of CHF in Russian patients was higher (frequency of FC IV CHF was 27 %), but LVEF was higher (28.2±8.1 %). ECG revealed more narrow complex QRS (151.9±34.3 ms) and lower frequency of right bundle branch block (3.9 %). Some differences in drug therapy were revealed. Thus, according to the register PANORAMA, the population of patients with CHF and implanted CRT and CRT-D devices in Russia is very different by many demographic and clinical characteristics from patient population included in main multicenter trials and registries abroad.
