Changes in morphofunctional cardiac parameters, immunological and emotional disorders in patients with heart failure

Background. Increased level of proinflammatory cytokines is an important factor of CHF progression. Chronic stress caused by emotional disturbances, increases neurohumoral system activation and expression of proinflammatory cytokines. Objective. To study the severity of depression and immunological abnormalities in patients with chronic heart failure, and to clarify the relationship of these indices with cardiac morphological and functional parameters. Materials and methods. Profile of cytokines (IL-1β, IL-6 and TNF-α), prevalence and level of depression (according to Hospital Anxiety and Depression Scale - HADS) were evaluated in 200 patients with CHF I–III FC (NYHA). Results. The relation between change of myocardial morphofunctional parameters, level of proinflammatory cytokines and severity of depressive disorders in patients with CHF was revealed. Patients with severe clinical manifestation of CHF had the highest prevalence and severity of depression. These patients showed the maximum production of proinflammatory cytokines.
