Evaluation of vascular remodeling in patients with coronary artery disease according to severity of chronic heart failure

Actuality. Structural and functional changes of blood vessels’ wall have important role in CHF’s pathogenesis. Aim. To study features of vascular remodeling in patients with stable angina FC II-III and II-III FC CHF according to the level of N-terminal fragment of brain natriuretic peptide (NT-proBNP) and its relationship to condition of myocardial and blood vessels’s collagen matrix. Materials and methods. The study included 40 patients with stable angina FC II-III in combination with CHF II-III FC. Patients’ examination included assessment of clinical status, echocardiography, measurement of pulse wave velocity (PWV), the level of NT-proBNP, volume sphygmopletismography. The condition of collagen matrix of vascular smooth muscle cells was evaluated by measuring the level of C-terminal telopeptide of procollagen type 1 (CTP 1) and the level of tissue inhibitor of matrix metalloproteinases (TIMP-1). Results. Vascular remodeling of large arteries in patients with stable angina FC II-III and CHF II-III FC predominantly with preserved EF is characterized by increased rigidity of the vascular wall, reduced elasticity and extensibility. Vascular remodeling in these patients is becoming more apparent with increasing of NT-proBNP level and FC of CHF, regardless of blood pressure level, severity of angina pectoris, presence and severity of cardiovascular risk factors.
