Main features of hemodynamic status of men and women with postinfarction heart failure, its relationship with psycho-emotional condition

Actuality. Gender features of hemodynamic and emotional state of patients with postinfarction CHF determine disease course, individual characteristics and prognosis. Aim. To study hemodynamic status according to echocardiography in men and women with postinfarction CHF, to evaluate its relationship with psycho-emotional condition. Materials and methods. The study included 344 patients, 257 men and 87 women with postinfarction CHF. The examination included echocardiography, 6 min walk test, psycho-emotional state assessement with Hamilton and Gotland scale, Spielberg test. Results. Heart chambers’ size according to echocardiography were significantly smallerr in women, due to peculiarities of these indicators calculation. Depressive disorders (DD) were detected in 31% of patients, with significant prevalence in women - 58% vs 21% in men. Analysis of groups of men and women, depending on the type of depressive disorder spectrum (isolated anxiety or DD) revealed no differences in echocardiography. We revealed the increase of DD severity with progression of CHF. Women had worse results of 6 min walk test, due to pronounced negative influence of anxiety and DD. Thus, gender differences of echocardiography, emotional status determine the necessity of a separate analysis of women and men to prevent detection of false dependencies and linkages. DD in patients with postinfarction CHF occur in 58% of females, which is associated with a worse exercise capacity. In male anxiety and DD did not affect the results of 6 min walk test. Hemodynamic status and psycho-emotional state independently influence on severity of postinfarction CHF, but among women emotional component’s effect on severity of CHF is predominant, whereas in males the determining factor is myocardial contractility.
