Borreliose infection: Role for myocardial remodeling in dilated and ischemic cardiomyopathy in an endemic region of Russia

Urgency. Up to the present there is no unified concept on the effect of Borrelia on the cardiovascular system. Aim. To determine the role of borreliose infection in processes of myocardial remodeling in patients with dilated and ischemic cardiomyopathy in an endemic region of Russia. Materials and methods. 26 patients with dilated cardiomyopathy (DCMP) and 64 patients with IHD after one or more myocardial infarctions were evaluated between 2005 and 2007. Patients with IHD were assigned to two groups. Group 1 included 32 patients with ischemic cardiomyopathy after Q-MI; group 2 included 32 patients with relatively normal heart dimensions according to echoCG. Epidemiological history was collected, indirect immunofluorescent reaction (IRIF) with a borrelioise diagnosticum was performed, and echoCG was recorded in all patients. Results. 34.6% of patients with DCMP residing in the endemic region had stage III borreliose infection; a connection between the fact of tick suction and HF development was demonstrated in 26.9% of cases. Stage III Lyme borreliosis was detected in 28.1% of patients with IHD and megalocardia living in the endemic region, which was significantly more frequent that in patients with IHD and normal heart dimensions (6.2%). In patients with megalocardia of  coronarogenic or non-coronarogenic genesis living in the endemic region, borreliose infection plays a significant role in heart remodeling, which requires serological testing for borreliosis to reveal indications for the etiotropic therapy, even in the absence of tick suction history.
