Peculiarities of biochemical parameters and androgenic status in patients with heart failure of different etiology

Urgency. Despite the blockade of neuro-humoral overactivation underlying the CHF pathogenesis, the condition of patients continues to impair in most cases, which perhaps warrants the necessity for more comprehensive studying other mechanisms of CHF development. Aim. Studying biochemical parameters and androgenic status in patients with decompensated CHF of different etiology. Materials and methods. 64 men (mean age, 42.46 ± 2.5 years) with decompensated CHF were evaluated. Patients were allocated to two groups based on the CHF genesis: group 1 including 22 patients with ischemic myocardiopathy (IMP) and group 2 including 42 patients with dilated myocardiopathy (DMP). In addition to clinical evaluation, biochemical tests including those for blood testosterone and luteinizing hormone as well as questioning using a special scale were performed. Results. Almost a half (45.4%) of patients with DMP, despite their young age, had an androdeficient status of different severity. Biochemical tests revealed increases in bilirubin, hepatic transaminases (ALT, 17%; AST, 7.7%), urea (11%) and creatinine (8.5%) as compared to patients with IMP, associated with decreases in parameters of the lipid spectrum. Therefore patients with decompensated CHF have a set of metabolic disorders accompanied by development of androgen-deficient status. These disorders are more pronounced in DMP.
