Blood levels of some cytokines in thyrotoxic myocardiopathy

Urgency. Studying the pathogenesis of heart injury in chronic iron-deficient anemia (IDA) is still an urgent problem due to the high incidence of anemic myocardial dystrophy. Aim. Studying parameters of structure, LV and RV diastolic function, LV systolic function and central hemodynamics in patients with IDA in relation to severity of disease. Materials and methods. Study enrolled 82 patients (68 females 14 males) with degree 1–3 chronic posthemorrhagic IDA. Echocardiographic examination was performed according to a standard method to determine ventricle dimensions, end systolic and end diastolic volumes, ventricular wall thickness and parameters of heart systolic function. Parameters of ventricular diastolic function were evaluated by the transmitral flow using Doppler echoCG. Results. Patients with IDA showed signs of LV myocardial hypertrophy, primarily at the expense of interventricular septum (IVS), with increased LV myocardium weight (LVMW), moderate chamber dilation, as well as increases in most parameters of central hemodynamics in patients with mild anemia and decreases in these parameters in patients with moderate and severe anemia. Impaired myocardial diastolic function was observed in all degrees of disease with most pronounced ones in LV. Extent of structure-activity heart disorders increased as anemia severity grew. Therefore the echoCG examination with dopplerohgraphy may be used in patients with IDA for early detecting and specifying the nature and degree of myocardial structure-activity disorders and also for decision about the tactics for therapy in myocardial dystrophy.
