Urgency. Symptom similarity complicates diagnostics of CHF in patients with chronic renal disease (CRD). Aim. To study a possibility for using brain natriuretic peptide (BNP) as a diagnostic marker for CHF in patients with pre-dialysis CRD. Materials and methods. 37 patients with pre-dialysis CRD were evaluated. Clinical and functional signs of CHF including the level of BNP were determined. Results. Signs of CHF were found in 81% of patients with pre-dialysis CRD including 67.7% with isolated diastolic CHF and 13.7% with a combination of diastolic and systolic CHF. The CHF signs documented by BNP measurement in pre-dialysis patients were symptoms of cardiac abnormality which could be designated as renal cardiomyopathy. Diagnostic levels of BNP≥100 pg/ml can be used as a marker for CHF in patients with pre-dialysis CRD.
