Disorders in the cytokine system and possibilities of their correction using β-adrenoblockers in patients with CHF

Urgency. One of the achievements in up–to–date cardiology is establishing the role of cytokine system activation in the pathogenesis of CHF. It is of current importance to study peculiarities of CHF pathogenesis and course in patients with the activated cytokine system as well as peculiarities of using β–adrenoblockers as modulators of the cytokine system in such patients. Aim of study: Optimization of drug therapy based on evaluation of the cytokine system in patients with CHF which has complicated the course of IHD, and development of pharmacotherapeutic approaches to its correction. Material and methods. To evaluate the state of cytokine system, levels of tumor necrosis factor (TNF–α) and interleukin–6 (IL–6) were measured in 80 patients with I–IV FC CHF using the enzyme immunoassay. Results. Comparing the extent of clinico–instrumental signs of CHF and plasma levels of TNF–α and IL–6 revealed a significant correlation between CHF degree and TNF–α concentration. Long–term therapy with bisoprolol 5–10 mg/day allowed an efficient control over the activity of cytokine system in patients with CHF, which had complicated the course of IHD. The treatment resulted in decreases of plasma TNF–α and IL–6 by 86.7 and 89.2%, respectively.
