Role of fosinopril in prevention of chronic heart failure in patients with arterial hypertension

Urgency. The efficacy of ACEI as the means for primary prevention of cardiovascular complications in AH is presently well demonstrated. The use of fosinopril in preventing progression of cardiovascular remodeling has been addressed insufficiently until recently. Aim: Studying the effect of the ACEI fosinopril on remodeling, diastolic filling of both ventricles, myocardial ectopic activity, heart rhythm variability, and microcirculation in patients with AH. Materials and methods. Study was carried out as a part of the FAGOT program. Seventy-eight patients with 1-2 degree AH were evaluated. The 24-hour BP monitoring, echoCG, Holter ECG monitoring, and Doppler laser flowmetry were performed. Results. In patients with AH, fosinopril slowed down the progression of cardiovascular remodeling; provided normalization of structure and geometry cardiac parameters, ventricular diastolic filling and microcirculation; decreased the sympathetic tone of autonomic nervous system and myocardial ectopic activity and thereby reduced the risk of sudden arrhythmic death.
