Should we blindly follow guidelines on treatment of heart failure based on results of international clinical trials? Significance of the study SENIORS for the Russian population of patients with CHF.

Epidemiological studies have shown a recent, slight but statistically significant decrease in death rate from HF in the population. A major cause for this beneficial phenomenon is the increasingly extensive introduction of ACEI and β-blockers, the drug classes «tested» in clinical trials (CT), to routine clinical practice. However for a more substantial improvement of prediction for the entire population of patients with CHF, it is necessary to rely more on data from studies performed on «typical representatives» of the population. For routine practice in Russia, patients with CHF and preserved myocardial systolic function are such «typical» ones. Peculiarities of this CHF form such as association with hypertension, high vascular resistance and myocardial stiffness imply a high efficacy of nebivolol, a β-blocker with NO-mediated vasodilatory properties. The study SENIORS became the first European study which has demonstrated a possibility of efficient therapy using nebivolol in such patients.
