Myocardial metabolism in chronic heart failure and current abilities of metabolic therapy

The clinical CHF syndrome is associated with pronounced myocardial metabolic dysfunction. Review provides pathophysiological characteristics of major aspects of myocardial metabolic remodeling in HF including disorders of myocardial energy metabolism, changed activity of intracellular regulatory enzymes, disorders of cardiomyocyte ion homeostasis, metabolic pathways leading to apoptosis, and metabolic activity of myocardial interstitium. Review also considers prospects for clinical use of pharmacological modulators of myocardial metabolic dysfunction in CHF.
